TRANSPORTATION CHARGES

Payment is due before the trip begins. All charges are payable in advance unless credit is established
with CGreen Transport, LLC prior to the trip. Method of payment shall be cash, certified check, VISA
or MasterCard. NO personal checks will be accepted (this is subject to CGreen Transport, LLC’s
discretion). A fee of $50.00 will be charged for personal checks returned for insufficient funds or stop
payments. Accounts delinquent over 30 days will be charged 20% of the total amount due. If payment
is not received after 60 days from date of service, CGreen Transport, LLC will seek legal recourse.
Delinquent accounts will then be subject to court costs and legal fees.

Tolls, fees and special charges for permits and/or entrance fees will be included in the cost of said trip.

Deposit and Refund Policy: CGreen Transport, LLC will require a deposit on all reservations placed. A
deposit fee of $25.00 and/or 50% of the total trip cost will be required upon placing said reservation if
not paid in advance. A minimum of 48 hours is required to make a reservation that includes the pick-up
date, pick-up time, pick-up address and drop-off address. A refund will be issued if Passenger(s)
cancels said trip twenty-four (24) hours before said pick-up date and pick-up time that was reserved by
Passengers(s). Cancellation request must be in writing. Refunds will be issued by check within thirty
(30) days after receiving written cancellation request.

Cleaning and damage charges: CGreen Transport, LLC, in its reasonable opinion, may charge any
Passenger who soils and/or damages Carrier’s vehicle in which said vehicle is rendered out-of-service
due to bodily fluids or other hazardous conditions which may place other Passenger(s) in harm. If said
vehicle is left dirty, torn or damaged or in an unsafe condition, a cleaning and/or damage fee of
$150.00 will be charged to said Passenger.

 

RULES AND REGULATIONS

CGreen Transport, LLC (CGT) is not responsible for any lost or stolen property. All bags and personal
property shall be carried by Passenger.

For bags that are oversized and/or weigh over 50lbs., an additional fee may be charged. For each
oversized or “over 50lbs.” bag, the Passenger will be charged $25.00 per item.

If due to CGT’s negligence, liability for loss of baggage will be limited to $25.00 per bag.

If any person(s) is under the influence of liquor or drugs, or is incapable of taking care of himself or
herself, or shows conduct which is objectionable, CGT shall have the right to refuse transportation to
said individual.

NO ANIMAL(s) OF ANY TYPE WILL BE CARRIED -- EXCEPTION: Passenger(s) who will be
accompanied by a service animal shall not be denied transportation as stated by The Americans with
Disability Act (ADA).

When delays caused by Acts of God, public enemies, authority of law, quarantine, perils of navigation,
breakdowns, bad conditions of the roads and blizzards, or any condition beyond its control occur, CGreen Transport, LLC will be held harmless of all damages to its Passenger(s). CGT will maintain the
schedule agreed upon by its agents or employees with its Passenger(s), but arrival and departure times
are not guaranteed.

If, in CGreen Transport, LLC’s opinion, it is inadvisable to operate its vehicle from place of origin or
reroute, CGT will not be held liable, or caused to be held liable, for any damages for any reason.

Under Virginia law, service to the general public must be provided without discrimination and CGT
cannot refuse service without good cause. All Passenger(s), while riding in CGT’s vehicle(s), shall
use/wear the Safety Seatbelt(s) at all times.

All Passengers(s), while riding in CGT’s vehicle(s), shall be advised by signs displayed in its vehicle(s)
that NO SMOKING, DRINKING, AND EATING.

CGT is not required to furnish car seats for children under the age of 8. It’s the sole responsibility of
parent(s) to supply such car seats. For any child that does not have the appropriate car seat, CGT has
the legal right to refuse such trip at the start of the trip for the safety of its Passenger(s).
